Request Fatal Crash Data

The WTSC manages the Fatality Analysis Reporting System (FARS), a census of all traffic-related fatalities occurring in Washington State. If you would like to request fatal crash information from the WTSC that is not already available from our website materials, please complete this form and provide as much information about your request as possible. Requests are generally completed in the order they are received. We strive to complete all requests within ten business days.

Please be as specific as possible. Be sure to include details such as age ranges, collision outcomes, specific contributing circumstances or traffic safety topics, road types, etc. The WTSC is the data steward of fatal collision information. Depending on the nature of your request, you may be referred to a more appropriate steward agency for the information you are requesting.
